In this episode of the Primary Care Knowledge Boost (PCKB) podcast, doctors Lisa and Sara talk to doctors Joanna Bircher, Nikesh Vallabh and Fran Fitton, along with SWAN PCN manager and transformation Lead, Shelley Ryder about proactive care projects focused on dementia care.
They talk about what proactive care is and discuss some example projects involving dementia care which Shelley and Fran are involved in.
The aim of the podcast is to give people a flavour of what the design and implementation of proactive care projects could entail in general practice.
A further podcast is planned later in the year with more examples of projects and how they got off the ground.
