Survey aims to understand how primary care views its role in supporting the design and delivery of the GM Live Well model.

The views of the primary care workforce are being sought to help shape the future of Greater Manchester Live Well.

The Live Well model aims to grow the support, control, connections and resources people need to lead healthy, happy lives.

Live Well is a city-region wide movement focused on tackling health, social and economic inequalities by changing how we work together across public services, voluntary organisations and communities.

It’s about ensuring people in every neighbourhood can access the right support, at the right time, in the right place.

This includes integrated access to financial, employment, wellbeing, health and social support through Live Well centres, spaces and offers, backed by a connected and proactive workforce.

This approach builds on what’s already working across our localities and aims to grow consistent, community-led, preventative models of support – reducing reliance on crisis services and improving outcomes for individuals and communities.

As key partners, everyone who works in primary care is invited to take part in a survey to understand the existing work in primary care and how it aligns with Live Well principles.

Your responses will help to better align Live Well delivery with the priorities of primary care and identify opportunities to amplify neighbourhood-level initiatives, enable more proactive care, and support the Live Well workforce to work across sectors.

This will particularly benefit people with complex needs, those economically inactive due to ill health, and patients who frequently present with social rather than medical issues.

The closing date for the survey is Tuesday, 24 June.
