Join our online workshop and get involved with the GM Live Well model.
Primary care colleagues in Greater Manchester are invited to an online workshop about the Live Well model.
The session is open to anyone working in community pharmacy, dentistry, general practice and optometry, and will help you to understand the GM Live Well model and what it could mean for primary care.
It will be hosted by Dr Jaweeda Idoo, Clinical Lead for Live Well, NHS Greater Manchester (NHS GM), and Helen Ibbott, Interim Director of Operations, Greater Manchester Primary Care Provider Board (PCB).
You will hear about:
- what the Live Well model is
- how it is currently working across GM
- the engagement that has been taking place across the four primary care disciplines
- how primary care could be engaged in the continued development of the approach
There will be an opportunity to share your experience of person and community-centred, and integrated neighbourhood approaches to date, as well as ask any questions.
You can choose either Thursday, 5 June, 12.30pm to 1.30pm or Wednesday, 11 June, 12.30pm to 1.30pm.
