A handy toolkit has been launched to help Primary Care Networks develop new models of proactive and personalised care in their communities.

The toolkit features the best resources from a programme run by Greater Manchester Primary Care Provider Board (GMPCB) and NHS Greater Manchester (NHS GM), in partnership with Peak Health Coaching.

The training programme equipped PCNs across Greater Manchester to take a health coaching and Quality Improvement approach. They introduced proactive and personalised care models aimed at helping patients with specific conditions.

As a legacy of the programme, the PCB has captured hours of expertise and training into a practical guide for teams and organisations to use and apply to any long-term health condition.

The toolkit highlights why proactive care is important, what you need to consider and how to help your communities live well for longer.

It features ‘how-to’ videos on data and workforce, handy QI templates, an explanation of the health coaching methods used and case studies from PCNs who took part in the programme.

Programme manager, Andrew Binnie, said: “We know that by being proactive, you can help to prevent and manage long-term health issues, especially when the care is personalised to an individual or community.

“This type of approach in Greater Manchester will help us support people to live healthier, more independent lives. It will also contribute to reduced demand on healthcare services – all of which are highlighted in the Greater Manchester Primary Care Blueprint and the NHS 10 Year Health Plan.

“The programme we ran over two years received such a positive response from our PCNs, and attracted interest from all over the country, that we wanted to capture the ‘best bits’ and share them with others. Primarily, this is a guide for Greater Manchester PCNs, but I’m sure it will be useful to anyone wishing to adopt a proactive and personalised approach to healthcare.”
